My mom stopped feeling thirsty at 72. I did not know that was possible. I thought thirst was universal, like breathing or blinking. But aging changes everything. The thirst mechanism weakens. The kidneys become less efficient. And by the time a senior feels thirsty, they are already dehydrated. Sometimes dangerously so.

I learned this the hard way. Last summer, during a heat wave in Portland, my mom collapsed in her garden. She was watering plants at 10 AM, temperature 88 degrees. Not extreme. But she had not had water since breakfast at 7 AM. She felt fine. She did not feel thirsty. Then she felt dizzy. Then she was on the ground.

The ER said she was severely dehydrated. Sodium levels elevated. Blood pressure low. She needed two liters of IV fluids. And she kept saying, "But I was not thirsty." That is the trap. Seniors do not feel thirst the way younger people do. Their bodies do not send the signal. Or the signal is so weak that it is ignored.

I researched this after the incident. Studies show that adults over 65 have a 40% reduction in thirst sensation. Forty percent. That means they need to drink water before they feel thirsty. They need schedules, reminders, and habits. Not intuition. Because their intuition is broken.

I made changes. I bought my mom a 32-ounce water bottle with time markers. Drink by 9 AM. Drink by 11 AM. Drink by 1 PM. It is not elegant. But it works. She finishes the bottle most days. I also added electrolyte packets to her routine. Not sports drinks — too much sugar. Just electrolyte powder in water. Because seniors do not just lose water. They lose sodium, potassium, and magnesium. And plain water does not replace those.

Dehydration in seniors is a leading cause of hospitalization. It causes confusion, falls, urinary tract infections, and kidney problems. And it is preventable. Not with thirst. With intention. With structure. With someone caring enough to notice that the thirst mechanism is broken and fix it anyway.

If you have an elderly parent, do not ask if they are thirsty. They will say no. Give them water anyway. Make it easy. Make it routine. And check their urine color. Pale yellow is good. Dark yellow is dehydration. It is not invasive. It is not complicated. It is just love, measured in ounces.

I also learned that medications make dehydration worse. My mom takes a diuretic for blood pressure. It makes her kidneys dump water. She also takes a medication that causes dry mouth as a side effect. She does not feel thirsty because her mouth is always dry — but that dryness is from medication, not hydration status. She was chronically dehydrated and had no idea. Her doctor adjusted the diuretic dose and added a saliva stimulant. Her hydration improved within a week.

The lesson is that elderly hydration is not just about drinking water. It is about medications, kidney function, and the broken thirst mechanism. It requires a holistic approach. Water bottles, electrolytes, medication reviews, and someone paying attention. I pay attention now. I call my mom every morning and ask if she finished her water bottle. She thinks I am nagging. I think I am keeping her out of the ER. We are both right.

I also started tracking my mom's fluid intake with a simple chart. Eight ounces at breakfast. Eight at mid-morning. Eight at lunch. Eight at mid-afternoon. Eight at dinner. Eight before bed. That is 48 ounces. Plus the water in her food — soup, fruit, vegetables. She is now consistently hydrated. Her energy is better. Her confusion episodes — which we thought were early dementia — have decreased. Dehydration mimics dementia. That is a terrifying fact. And it is fixable with water.
